Age Well at Home – Senior Wellness Connection Program
Age Well at Home – Senior Wellness Connection is designed to support seniors with grocery delivery, medical transportation, friendly visiting, and meal delivery services. The services are available to all senior 55+ and low income in all of Metro Vancouver. S.U.C.C.E.S.S. are currently focusing on enrolling new clients especially for meal delivery services and are looking to serve 20 seniors starting as early as July.
How to Sign Up: Signing up is easy! Simply fill out the referral form, or if you are interested in the programs yourself, call Michelle at 604-374-0451. She is fluent in English, Cantonese, and Mandarin.
Meal Delivery Program (Cost: Free)
Are you a senior 55+ and low-income? Do you crave a hot, delicious, culturally appropriate, and nutritious meal delivered right to your door? Look no further! The Age Well – Senior Wellness Connection Meal Delivery Program is here to serve you. Enjoy one free, Asian hot meal every other Saturday afternoon, prepared by dedicated volunteers in a professional kitchen. The menu changes every delivery.
Grocery Delivery Program (Cost: Only for the groceries ordered)
S.U.C.C.E.S.S. partners with grocery stores (T&T, Walmart, Superstore) to deliver groceries once a week. Your order must be at least $20, but there is no maximum. Delivery fees are covered by S.U.C.C.E.S.S. and clients only need to pay for the cost of the items and associated taxes.
Friendly visiting (Cost: Free)
The Friendly Visiting Program enhances the well-being of seniors by fostering meaningful connections within their community. Volunteers are thoughtfully matched with seniors to visit seniors regularly, sharing conversations, hobbies, and activities that bring joy and fulfillment to both parties. This volunteer service is designed to be accessible to all seniors, without any financial burden.
Transportation to medical appointments (Cost: Free)
Volunteer drivers provide safe and courteous rides for seniors to medical appointments. S.U.C.C.E.S.S. requires the senior to contact them by email or phone 2 weeks prior to appointment time.The transportation provided by volunteer drivers is free of charge
